Job Title: Community Health Integration (CHI) Care Coordinator

Location: Dubai, UAE
Working Hours: 5:00 PM–2:00 AM, Dubai time
Work Arrangement: Full-time, office-based only
Remote Work: Not available

We are hiring a compassionate and organized Community Health Integration Care Coordinator to support patients receiving healthcare services in the United States.

The successful candidate will communicate with patients, identify health-related social needs, coordinate community resources, and follow up on care plans under the supervision of the clinical team.

Key Responsibilities:

* Speak with patients by telephone and video
* Identify barriers involving transportation, food, housing, medications, and access to care
* Connect patients with appropriate community organizations and support services
* Coordinate appointments and communicate with healthcare providers
* Follow up with patients and document their progress
* Maintain detailed and accurate electronic health records
* Escalate urgent clinical or safety concerns to the appropriate supervisor
* Follow patient-privacy, confidentiality, and company compliance procedures
* Complete assigned patient-contact and documentation requirements

Requirements:

* Excellent spoken and written English
* Strong communication, listening, and problem-solving skills
* Compassionate and professional when working with older adults
* Experience in healthcare, social work, case management, patient support, or care coordination is preferred
* Familiarity with the U.S. healthcare system is an advantage
* Comfortable using electronic records, email, telephone, and video platforms
* Available to work from 5:00 PM to 2:00 AM in our Dubai office
* Reliable, organized, and able to manage multiple patient cases
* Must work entirely from the office; this is not a work-from-home position

Education:

A degree or diploma in social work, psychology, nursing, healthcare administration, or a related field is preferred. Relevant care-coordination experience may also be considered.
